Shift Details: This is a full-time, Monday through Friday position with daytime hours. While schedules offer flexibility to accommodate patient needs, RN Case Managers participate in a rotating on-call schedule, which includes evenings and weekends. Travel within our service area is required. Who You Are: • Current Registered Nurse (RN) license in the state of Indiana. • Minimum of 1-2 years of nursing experience (home health or acute care preferred). • Knowledge of OASIS documentation and Medicare home health regulations preferred. • Strong clinical assessment, critical thinking, and problem-solving skills. • Excellent organizational and communication skills. • Ability to work independently and manage a caseload effectively. • Valid driver’s license, reliable transportation, and proof of auto insurance.
