Physical Therapist (PT) Rate: $45 - $52.24/hour (depending on experience) Shift: Full-Time Schedule: Days Taconic Rehabilitation & Nursing at Beacon is looking to add a Physical Therapist (PT) to its rehabilitation team. RESPONSIBILITIES: • Evaluate residents and develop individualized physical therapy treatment plans • Establish short- and long-term rehabilitation goals and monitor resident progress • Provide skilled therapy services to improve mobility, strength, balance, endurance, and functional independence • Supervise Physical Therapist Assistants (PTAs) in accordance with New York State regulations • Complete required documentation, evaluations, care plans, and billing records accurately and timely • Collaborate with physicians, nursing staff, families, and interdisciplinary team members to support resident outcomes • Provide resident and family education regarding therapy goals, adaptive equipment, and home exercise programs • Recommend and coordinate durable medical equipment (DME) and discharge planning needs • Participate in care conferences, staff education, orientation, and quality improvement initiatives • Maintain therapy equipment, supplies, and treatment areas • Perform additional duties as assigned REQUIREMENTS: • Master's or Doctorate Degree in Physical Therapy from an accredited program approved by the American Physical Therapy Association • Valid New York State Physical Therapist License required • Newly licensed Physical Therapists encouraged to apply • Skilled nursing facility experience preferred • Strong clinical, documentation, communication, and problem-solving skills • Ability to work collaboratively within an interdisciplinary healthcare team • Commitment to providing compassionate, resident-centered care BENEFITS: • Paid Time Off (PTO) for Full-Time and Part-Time • Weekly or Same-Day Pay Options • Health, Dental, and Vision Insurance • Life Insurance • Employee Referral Bonus Program • Retirement Saving Options
