Delivering Solutions that Matter: • Provide hands-on physical therapy treatments and exercises under the direction of a licensed Physical Therapist to improve residents' strength, mobility, balance, and functional independence. • Assist residents with transfers, ambulation, positioning, and therapeutic activities while promoting safety, dignity, and confidence. • Monitor residents' progress and communicate changes in functional status, tolerance, or condition to the Physical Therapist and interdisciplinary care team. • Accurately document treatments, resident responses, and progress in accordance with care plans, facility policies, and regulatory requirements.
