Job Duties: Formulate and execute discharge plans. Assist in the admission process. Provide psycho-educational experiences. Communicate with support systems (families, friends, group homes). Communicate with providers. Coordinate placement. Serve on treatment team. Collect and submit Medicaid prior approval information. Maintain documentation. Qualifications: A minimum of a Bachelor’s Degree in Social Work and passage of the Social Work Licensure Test with Social Work License current and in good standing. Valid Mississippi Driver’s License
