Full-time Home Health Physical Therapist in Springfield, Missouri. This role offers a Monday-through-Friday schedule with no weekend requirement and supports candidates without prior home health experience to grow within the specialty. Key Responsibilities: • Perform comprehensive patient evaluations and develop individualized treatment plans • Conduct start-of-care assessments and complete OASIS documentation accurately • Provide routine home-health therapy visits focused on improving strength, balance, mobility, and independence • Complete patient recertifications and reassessments as required • Educate patients and caregivers on therapeutic exercises, mobility strategies, and home safety • Collaborate with physicians, nurses, case managers, and interdisciplinary healthcare teams • Monitor patient progress and modify treatment plans to maximize outcomes • Maintain timely and accurate clinical documentation using Matrix EMR • Deliver compassionate, patient-centered care in a home-based setting Qualifications: • Doctorate or Master's degree in Physical Therapy from an accredited program • Active Missouri Physical Therapist (PT) license • Strong communication skills • Strong organizational skills • Strong time-management skills • Ability to work independently and manage a daily patient schedule Preferred Skills: • Current CPR/BLS certification • Experience with OASIS documentation and Matrix EMR
