You'll collaborate with physicians and an interdisciplinary care team to develop individualized treatment plans that help patients achieve their rehabilitation goals. • Evaluate patients and develop individualized treatment plans. • Provide therapeutic exercise, manual therapy, gait training, mobility, and functional rehabilitation. • Monitor patient progress and modify treatment plans as appropriate. • Educate patients and caregivers on treatment plans and home exercise programs. • Collaborate with physicians and interdisciplinary care teams to optimize patient outcomes. • Maintain accurate EMR documentation and comply with hospital and regulatory standards. Qualifications EDUCATION, EXPERIENCE, TRAINING 1. Bachelor/ Masters/ Doctorate/Certificate degree in Physical Therapy 2. Current state Physical Therapy license 3.
