In this role, you will provide high-quality, patient-centered care in the comfort of patients' homes while working alongside a supportive interdisciplinary team. What We Offer • Competitive per-visit pay • Flexible scheduling • Electronic charting (Homecare Homebase – HCHB) • Supportive clinical leadership • Opportunities for professional growth and advancement • Mileage reimbursement Responsibilities • Provide skilled nursing care in patients' homes under the direction of the Registered Nurse and physician • Monitor patient condition, vital signs, and overall health status • Administer medications and treatments as ordered and within the LPN scope of practice • Accurately document all visits and care provided in Homecare Homebase (HCHB) • Communicate changes in patient condition to the supervising RN and physician • Educate patients and families on medications, disease management, and the plan of care • Ensure compliance with all federal, state, and agency regulations • Collaborate with the interdisciplinary team to achieve optimal patient outcomes Requirements • Active Michigan Licensed Practical Nurse (LPN) license in good standing • Graduate of an accredited practical nursing program • Current BLS/CPR Certification • Valid driver's license, reliable transportation, and current auto insurance • Strong clinical, communication, and documentation skills • Ability to work independently in a home health setting Preferred Qualifications • Home health or hospice experience • Experience with Homecare Homebase (HCHB) or other electronic medical record systems • Wound care experience • Bilingual candidates are encouraged to apply Why Join Us?
