The RN will be responsible for conducting patient assessments, developing individualized care plans, supervising caregivers, and ensuring high-quality care is delivered in accordance with state regulations and company standards. Qualifications • Active and unrestricted Registered Nurse (RN) license in the applicable state. • Minimum of 1 year of nursing experience preferred. • Previous home care or home health experience preferred. • Strong assessment, communication, and organizational skills. • Ability to work independently and manage multiple patient cases. • Valid driver's license and reliable transportation. • Proficiency in electronic medical records (EMR) and clinical documentation.
