Nevada – Neurosurgery Spine Opportunity
• Nevada’s premier spine, neurosurgery, orthopedic, vein & neurology specialty group
• 62 MD/APP, 27 PT/OT
• Call Schedules – Trauma call (with 2nd call), practice neurosx call.
• Weekend and total call days distributed evenly amongst active call surgeons
• Hospital/ practice call are primarily fielded by neurosurgery Pas and escalated to surgeons as needed
• Trauma call primarily goes directly to surgeon on call and they decide if they need to go into the OR
• Fellowship training preferred but with strong residency program not required
• Standard clinic day is 12-16 patients per day depending on new patients vs follow ups
• Neurosurgeons work closely with neurosurgery PAs who primarily work the patients up to maximize surgeon time and efficiency
• Volume
• Organizationally all spine surgeons did 1600 cases in 2024 with neurosurgeons averaging 350 cases.
• Currently they have 2 ortho spine MDs with one looking to retire and one also doing general ortho. Plan is to move to neurosurgeons as we move forward with the ability to also take neurosurgery call
• Cases Performed
• Dependent on surgeon but typically 75-80% MIS, 20-25% complex <5% cranial unless a surgeon specifically drives cranial volumes
• Standard practice referrals are split evenly across surgeons
• Direct surgeon referrals go to that surgeon/dedicated PA
• Standard schedule is 3 days in clinic, 2 days surgical. A busy surgeon can do 2 days clinic with additional PA support and 3 days surgery
• Ideally our surgeons try to hit 30 surgeries per month
• Workload is split evenly between neurosurgeons
• Growing patient demand specifically in the Carson/Tahoe region
• Backfilling 1 departing neurosurgeon who is moving to a hospital employed environment (VA)
• Preparing for decreased production from 2 current spine surgeons nearing the tail end of their careers
• Salary/Benefits
• They are on a production based model with 55% overhead contribution and 45% direct pay for patient related revenues. 75% direct pay for trauma call, 25% overhead charge on trauma call revenues to support neurosurgery PA call support
• Neurosurgeon compensation has averaged $1.2M-$1.4M the last handful of years
• Neurosurgeons do receive a base “draw” against their productivity payouts for monthly bills. Range has historically been $300K-$400K depending on surgeon preference but is negotiable
• 1 year salary guarantee however neurosurgeons surpass salary guarantee quickly moving into the production model and direct call pay
• They are a production practice with surgeons getting a percentage of their monthly collections. Company is open to tiered overhead model increasing surgeon pay
• Additional ancillary profit share opportunities available after two years
• ASC shareholder opportunity available with separate distributions
• Company pays for Medical, Dental, Vision, and 401K contribution. Supplemental insurances (STD, LTD, etc) available at surgeon’s expense.
• Company pays for medical malpractice coverage, licensing fees, credentialing fees, etc
• Tail coverage typically covered by surgeon
• CME allowance annually
• Sign-on and relocation bonuses offered
• Standard starting PTO is 4 weeks + 1 week CME
• As surgeon gets established it is purely productivity model and time off is not monitored as long as baseline production is met
• CME is $3K annually and ability for additional CME funds with pre-approval
• Athena Health EMR
