Connected Health Care is partnering with a leading provider in Branson, Missouri to hire a compassionate Full Time Home Health Physical Therapist. The role offers a stable permanent position with flexible autonomy, allowing therapists to make a meaningful impact on patients in their homes. Key Responsibilities: • Perform comprehensive patient evaluations and develop individualized treatment plans • Conduct start of care assessments and complete OASIS documentation accurately • Provide routine home health therapy visits focused on improving strength, balance, mobility, and independence • Complete patient recertifications and reassessments as required • Educate patients and caregivers on therapeutic exercises, mobility strategies, and home safety • Collaborate with physicians, nurses, case managers, and interdisciplinary healthcare teams • Monitor patient progress and modify treatment plans to maximize outcomes • Maintain timely and accurate clinical documentation using Matrix EMR • Deliver compassionate, patient-centered care in a home-based setting Qualifications: • Doctorate or Master's degree in Physical Therapy from an accredited program • Active Missouri Physical Therapist (PT) license required • Strong communication, organization, and time management skills • Ability to work independently and manage a daily patient schedule Preferred Skills: • Current CPR/BLS certification preferred • Experience with OASIS documentation and Matrix EMR preferred but trainable Benefits: • Competitive Salary + Full Benefits
