As a Licensed Vocational / Practical Nurse you’ll contribute to our success in the following ways: • Follow each patient's individualized care plan and physician orders. • Monitor patient progress and report changes or safety concerns to the care team. • Help patients and caregivers learn self-care techniques to support their health. • Work with the RN Case Manager to update care plans as patient needs change. • Administer and document medications according to agency guidelines. • Communicate with physicians, patients, families, and the care team to ensure coordinated care. • Help prevent avoidable hospitalizations and emergency room visits through proactive patient care. • Deliver compassionate, high-quality care that supports positive patient outcomes.
