Education: • Doctor of Medicine (MD) or Doctor of Osteopathy (DO) or Doctor of Dental Surgery (DDS) or Doctor of Dental Medicine (DMD) or Doctor of Optometry (OD) or Doctor of Podiatric Medicine (DPM) required.
