What You Will Do: As a Home Health & Hospice CNA, you'll provide compassionate, hands-on care to patients in their homes, helping them remain comfortable, safe and as independent as possible. In this role, you will: • Visit approximately 5–6 patients per day within your assigned geographic territory. • Assist patients with daily living activities and personal care needs. • Support patient comfort, dignity, safety and overall well-being. • Build meaningful relationships with patients and their families through individualized care. • Observe and communicate changes in patient needs to the nursing team. • Collaborate with nurses and other care team members to provide coordinated, high-quality care. • Independently manage and coordinate your daily patient visits while remaining connected to a supportive team.
