Social Worker Overview • The Social Worker role is a contract position focused on supporting clients in an adult day care setting. • Collaborate with interdisciplinary teams to provide comprehensive assessments and care planning for clients. • Work closely with families to address psychosocial needs and enhance client well-being. • Provide education on older adults' rights and the needs of vulnerable populations. • Support dementia care programs and assist in memory impairment initiatives. • Ensure accurate documentation and compliance with care standards. • Facilitate team and family meetings to coordinate care effectively. • Contribute to the development of innovative care strategies for elderly clients. Social Worker Key Responsibilities & Duties • Conduct MDS and psychosocial assessments for clients in dementia care programs. • Coordinate care planning and ensure timely completion of documentation. • Facilitate team and family meetings to address client needs and concerns. • Educate families and caregivers on elderly rights and well-being. • Collaborate with interdisciplinary teams to enhance client care outcomes. • Support dementia care initiatives and memory impairment programs. • Maintain accurate records and ensure compliance with regulatory standards. • Develop and implement strategies to improve client psychosocial health. Social Worker Job Requirements • Master's degree in Social Work (MSW) from an accredited institution. • Valid New York State License in Social Work (LMSW). • Minimum of 1 year experience; 2 years preferred in dementia care. • Experience in adult day care settings is highly desirable. • Certified in Dementia Care is a plus. • Bi-lingual proficiency in English and Spanish is advantageous. • Strong written, communication, and organizational skills are essential. • Ability to work effectively in an interdisciplinary team environment.
