Social Worker Key Responsibilities & Duties • Conduct comprehensive MDS and psychosocial assessments for clients in dementia care programs. • Coordinate care planning and ensure timely completion of required documentation. • Facilitate team and family meetings to address client needs and provide education. • Collaborate with interdisciplinary teams to deliver high-quality care and support. • Educate families and caregivers on the rights and well-being of elderly clients. • Engage with family members and friends to provide comprehensive support for clients. • Ensure compliance with program standards and regulations in all documentation and care activities. • Participate in ongoing training and development to enhance dementia care expertise. Social Worker Job Requirements • Master of Social Work (MSW) degree from an accredited institution. • Valid New York State License in Social Work (LMSW). • Minimum of one year of experience, with two years preferred. • Five years of experience in dementia care is highly desirable. • Certification in Dementia Care is a plus. • Proficiency in English and Spanish is advantageous. • Experience in adult day care settings is preferred. • Strong written, communication, and organizational skills are essential.
