Social Worker Key Responsibilities & Duties • Conduct psychosocial and MDS assessments to evaluate client needs and develop care plans. • Coordinate care planning and ensure accurate and timely documentation. • Facilitate team meetings and family discussions to address client needs and concerns. • Educate families on dementia care and the rights of older adults. • Collaborate with interdisciplinary teams to enhance service delivery and program effectiveness. • Provide support for the Enhanced Memory Impaired program through specialized interventions. • Engage with family members and caregivers to ensure holistic care approaches. • Maintain compliance with state regulations and organizational standards in social work practices. Social Worker Job Requirements • Master's degree in Social Work (MSW) from an accredited institution. • Valid New York State License in Social Work (LMSW). • Minimum of one year of experience in social work; two years preferred. • Five years of experience in dementia care is highly desirable. • Bi-lingual proficiency in English and Spanish is a plus. • Certification in Dementia Care is advantageous. • Experience in adult day care settings is preferable. • Strong written, communication, and organizational skills are essential.
