Team Manager – Hospice Overview • The Team Manager – Hospice oversees nursing staff to ensure high-quality hospice care delivery. • Provides leadership and mentoring to nursing staff, fostering excellence in patient care. • Collaborates with interdisciplinary teams to align care with patient and family goals. • Supports adherence to program policies and state and federal regulations. • Schedules staff assignments to meet organizational and regulatory standards. • Monitors and evaluates nursing documentation for compliance and quality. • Participates in community outreach to promote hospice services. • Offers a hybrid work arrangement with remote flexibility. Team Manager – Hospice Key Responsibilities & Duties • Lead and mentor nursing staff to ensure consistent delivery of hospice care. • Collaborate with the Associate Director to schedule staff assignments effectively. • Empower RN Case Managers in pain and symptom management practices. • Monitor and evaluate nursing documentation for compliance and accuracy. • Participate in interdisciplinary team meetings to align care strategies. • Make community presentations to promote hospice services. • Ensure adherence to state and federal regulations in hospice care delivery. • Support orientation and training programs for new staff. Team Manager – Hospice Job Requirements • Bachelor of Science in Nursing (BSN) required; BS degree preferred. • Minimum of five years of hospice nursing experience required. • Supervisory or leadership experience preferred. • New York State RN licensure required. • National Board Certification in Hospice and Palliative Care (CHPN) preferred. • Proficiency in Word, Outlook, and Windows applications required. • Ability to supervise and mentor hospice nursing staff effectively. • Strong teamwork skills within interdisciplinary team environments.
