Job Title Physical Therapist Location Oakland, CA | Home Health Schedule / Job Type Contract | Days | Onsite / Field-Based Key Responsibilities • Provide skilled physical therapy services to patients across an assigned multi-city service territory in the Oakland and Richmond metro areas • Perform evaluations, re-evaluations, OASIS assessments, routine visits, and discharge visits as part of a home health caseload • Complete all required clinical documentation, including OASIS SoC, ReCertification, RoC, and discharge documentation, in accordance with agency and regulatory standards • Coordinate care through phone communication with patients and physicians as needed • Travel outside the primary service area and provide weekend coverage when required to meet patient visit expectations • Maintain willingness to see patients with communicable conditions, including Covid-positive and Covid PUI patients Required Qualifications • Current and active California Physical Therapist (PT) license • Current BLS certification • Valid driver's license, current auto insurance, and current vehicle registration • Up-to-date health clearances required: TB (annual), MMR, Varicella, HepB, TDap (within 10 years), and Flu (annual) • Successful completion of pre-placement drug screen and physical • Expected weekly productivity of at least 27 points per week; expected schedule of approximately 36 hours per week (productivity and hour expectations subject to caseload availability and applicable guarantee terms) Preferred Experience / Skills • Prior home health physical therapy experience • Familiarity with OASIS documentation and Medicare home health guidelines • Comfort managing an independent, field-based caseload across a multi-city territory Why Join Us • Opportunity to work in a dynamic home health setting serving a diverse patient population across the greater Oakland and East Bay area • All-inclusive rate structure designed to account for drive time, visit time, documentation, and patient/physician communication
