Job Title Physical Therapist Location Avondale, AZ | Outpatient Schedule / Job Type Contract | Full-Time | Days | 5 x 8-hour shifts | Expected 40 hours/week (32 hours/week during holiday weeks) | Onsite Key Responsibilities • Provide direct physical therapy services to patients in an outpatient setting • Complete required documentation accurately and in a timely manner in accordance with facility and regulatory standards • Evaluate patients, develop individualized treatment plans, and monitor progress toward goals • Collaborate with interdisciplinary team members to ensure continuity and quality of patient care • Maintain an active Physical Therapist license as required for the state of Arizona Required Qualifications • Active Physical Therapist license — a compact license may be accepted to begin, but a permanent Arizona license must be obtained promptly upon assignment start • Proof of license application must be provided within 5 business days of booking if permanent licensure is pending • Prior experience in an outpatient physical therapy setting • Ability to meet facility credentialing requirements prior to start • Candidates who have worked at this facility in a permanent or PRN capacity within the last year are not eligible for this assignment Preferred Experience / Skills • Previous travel therapy experience in an outpatient setting • Familiarity with outpatient documentation systems and workflows • Strong communication and patient engagement skills Why Join Us • Parking and dress code information provided upon start — equipment provided by the facility • Opportunity to work in a structured outpatient environment with consistent daytime scheduling
