Job Title Physical Therapist Location Fremont, CA | Home Health Schedule / Job Type Contract | Days | Field-Based / Travel Required Key Responsibilities • Provide physical therapy services to patients across an assigned multi-city service territory in the East Bay and Oakland/Berkeley areas • Conduct evaluations, OASIS assessments (Start of Care, Recertification, Resumption of Care, Discharge), routine visits, and re-evaluations • Manage a full patient caseload, meeting expected weekly productivity standards (minimum 27 points per week under applicable arrangements) • Coordinate care through phone communication with patients and physicians; documentation is included within the scope of each visit • Travel outside the primary service area and provide weekend coverage as needed to meet patient visit expectations • Must be willing to see COVID-positive and COVID PUI patients Required Qualifications • Current California Physical Therapist (PT) License • Current BLS Certification • Valid Driver's License, current auto insurance, and current vehicle registration • Up-to-date health documentation required: TB (annual), MMR, Varicella, Hepatitis B, TDap (within 10 years), Flu (annual) • Must pass pre-assignment drug screen and physical Preferred Experience / Skills • Prior home health physical therapy experience • Familiarity with OASIS documentation and home health visit workflows • Comfort managing a geographically distributed caseload across multiple cities Why Join Us • Opportunity to work across a diverse, multi-city territory serving a broad patient population in the greater East Bay area • Expected 36 hours per week available under applicable productivity arrangements, offering schedule consistency throughout the assignment
