Provide emotional support, resources, and solutions. • Make an impact. Coordinate personalized care plans, assessments, and social services while ensuring regulatory compliance. What You Bring • Master's in Social Work + 1 year of healthcare experience OR • Bachelor's in Social Work, Psychology, Sociology, or related field + 3 years of healthcare experience • At least 1 year of supervised healthcare experience • Social Worker license as required by state regulations • CPR certification • A passion for helping older adults live their best lives!
