Overview: Work Schedule - Saturday and Sunday are required (the other days of the week are flexible). Responsibilities: As a member of the multidisciplinary team, the Care Management Assistant provides discharge planning support to the Care Manager and Social Worker and supports patients and families / caregivers by reducing non-clinical barriers to discharge in the Inpatient and ED environments. Qualifications: Education • H.S.
