The LPN will provide direct patient care to an underserved population while working collaboratively with physicians, registered nurses, correctional staff, and other healthcare professionals. What you will be doing: • Administer medications and treatments as prescribed by healthcare providers. • Perform patient assessments and document findings accurately and timely. • Monitor and evaluate patient health status, reporting significant changes to the RN or provider. • Provide wound care, chronic disease management, and routine nursing interventions. • Assist with intake screenings, health assessments, and discharge planning. • Maintain accurate medical records in accordance with facility policies and regulatory requirements. • Respond appropriately to medical emergencies and urgent healthcare needs. • Educate patients on health conditions, medications, and treatment plans. • Collaborate with interdisciplinary healthcare and correctional teams to ensure continuity of care. • Adhere to infection control, safety, and security procedures at all times. What you must have: • Current, unrestricted Licensed Practical Nurse (LPN) license. • Current CPR/BLS certification. • Minimum of one (1) year of nursing experience in a Correctional Facility. • Strong clinical assessment and medication administration skills. • Ability to work effectively in a secure correctional environment. • Excellent communication, documentation, and critical thinking skills.
