Corning Center is hiring an Assistant Social Worker (SW) in Elmira, NY. DUTIES: • Maintaining and/or improving resident’s ability to control everyday physical needs and mental/psychosocial needs • Identifying individual social and emotional needs • Developing and maintaining individualized care plans • Participating in the planning of the resident’s admission, community re-entry or transfer to another facility • Participating in interdisciplinary care plan meetings to ensure treatment of the social and emotional needs of the resident REQUIREMENTS: • Long Term Care/ Subacute Experience Required • Excel/Word experience - Recommended About Us: Corning Center for Rehabilitation and Healthcare is a 120-bed skilled nursing facility located in Corning, New York, and is designed for individuals with skilled nursing needs, postoperative recuperation, or complex medical care demands, as well as chronically ill individuals who can no longer live independently.
