JOB SUMMARY: Position responsible for participating in the coordination of all systems / services required for an organized, multidisciplinary team approach and assures quality, cost-effective care for the identified patient population. MINIMUM QUALIFICATIONS: 1.Education/Specialized Training/Licensure:Masters Degree in Behavioral Sciences or Social Work with Licensure (LPC or LMSW). An LCDC is preferred. 2.Work Experience:Experience in a hospital/health setting preferred Experience working with patients with chemical use/abuse/dependency counseling and treatment preferred. 3.Equipment Operated:Computer SPECIAL REQUIREMENTS: 1.Communication Skills: Above Average Verbal (Heavy Public Contact) Bilingual Skills Required: No Languages: Spanish preferred Writing /Composing (Reports) 2.
