Home Health Physical Therapist Job Summary We are seeking a dedicated and compassionate Home Health Physical Therapist to provide high-quality therapy services to patients in their homes across the region. This position requires strong clinical skills and the ability to work independently while ensuring optimal patient care. Shift Information • Shift: Days, Monday-Friday • Hours: 8:00 AM – 5:00 PM Key Responsibilities • Conduct evaluations and assessments of patients. • Develop and implement individualized treatment plans. • Provide skilled physical therapy services in patients’ homes. • Monitor patient progress and adjust treatment plans as needed. • Collaborate with healthcare team members to ensure comprehensive patient care. • Document patient care and maintain accurate records. Qualifications • Valid California Physical Therapist License. • Minimum of 2 years of experience in physical therapy, with home health experience preferred. • Current Basic Life Support (BLS) certification from the American Heart Association (AHA) or American Red Cross (ARC). • Strong interpersonal and communication skills. • Ability to work independently and manage time effectively. • Willingness to travel; approximately 25% travel is required for this position. Additional Information Mileage reimbursement is provided at the General Services Administration (GSA) rate.
