Coordinates with psychiatric and medical providers. Provide rehabilitation and support functions to assist in treatment, medication education, support, consultation to families, and crisis intervention. Individual and group interactions with individuals who have a serious and persistent mental health disorder. Community-based with services provided in the home or other community settings.
