Practice Act. Licensure/Certification/Registry: - Illinois eligible or licensed Physical Therapist. - CPR certification is required. Other Knowledge/Skills/Abilities: - Universal Precautions Category I. - Must possess valid Illinois driver’s license and must be deemed as an acceptable driver in accordance with the MHS Fleet Safety Policy (five year MVR will be required). Must show proof of automobile insurance. Pay Transparency $37.43 - $58.02 Additional compensation in the form(s) of participation in our annual incentive program are dependent on role. Pay is based on factors such as work experience, education, certification(s), training, skills, and competencies related to the role.
