Overview As the Social Services Director you will identify and provide for each resident’s social, emotional and psychological needs, and the continuing development of the resident’s full potential during his/her stay at the facility and to assist in the planning for his/her discharge. How you Will make a difference • Assist residents and their families with social, emotional, and family issues. • Provide timely and accurate completion of documentation needed to comply with Facility, State, and Federal policies. • Participate in resident care planning. • Participate in the development of Discharge planning and prepare resident for the transition. • Serve as a general source of information for residents and their families. What you Need to make a Difference • Bachelor’s or Masters Degree in Social Work or Psychology. • 3+ Years Experience working in Social Services, directly with individuals, preferably in a healthcare setting. • High level of professionalism and ability to maintain confidentiality.
