Licensed Clinical Social Worker Overview • The Licensed Clinical Social Worker (LCSW) will provide comprehensive psychosocial assessments and therapeutic services to individuals and couples in a clinical setting. • Collaborate closely with psychiatrists and other care providers to ensure cohesive treatment plans and patient care. • Prepare and maintain detailed documentation including assessments, care plans, updates, and discharge notes in compliance with organizational policies. • Participate in bi-monthly group supervision sessions to enhance professional development and clinical practice. • Educate patients about the Patient-Centered Medical Home (PCMH) program, promoting its values and goals effectively. • Attend required in-service education programs to stay updated on clinical practices and standards. • Maintain active licensing status in New York State by completing continuing education requirements as mandated. • Travel between clinic sites as needed to meet scheduling and patient care demands.
