This is a field-based healthcare sales and community education position where you will spend your day: • Visiting hospitals, skilled nursing facilities (SNFs), assisted living communities, physician offices, and palliative care clinics • Building trusting relationships with physicians, discharge planners, case managers, social workers, and nursing staff • Educating referral sources, patients, and families on Guardian Angel Home Care’s compassionate hospice care services • Guiding families through delicate transitions and ensuring timely admissions for patients requiring end-of-life care What You'll Actually Be Doing (Daily Expectations) Full-Time: 12–15 in-person stops per day Part-Time: 6–8 in-person stops per day A stop may include: • Meeting with hospital discharge planners and case managers to identify patients appropriate for hospice care • Educating medical professionals and families on the benefits of early hospice intervention and symptom management • Collaborating with our clinical intake team to ensure seamless, prompt transitions from hospital or facility to home hospice care • Maintaining strong, ongoing relationships with existing referral partners while continuously developing new referral streams About Us Guardian Angel Home Care is a leading healthcare provider serving patients throughout Michigan and multiple states nationwide.
