Interim Master of Social Work (MSW) Key Responsibilities & Duties • Conduct and document comprehensive psychosocial assessments for patients. • Educate patients and families on rights and encourage active participation in care decisions. • Provide liaison services and referrals to community resources, including medication assistance programs. • Identify mental health needs and deliver counseling and referrals as indicated. • Collaborate with interdisciplinary teams to ensure seamless patient care transitions. • Coordinate placements with healthcare facilities such as LTACs, SNFs, and assisted living facilities. • Act as a resource for patients, families, and healthcare teams regarding emotional and behavioral issues. • Facilitate custodial care arrangements and acute rehab placements as needed. • Promote continuity of care by connecting patients to appropriate community services. Interim Master of Social Work (MSW) Job Requirements • Master of Social Work degree from an accredited graduate school of social work. • Minimum of three years of clinical social work or allied health practice. • Experience in hospital or medical settings preferred. • Strong knowledge of community resources and crisis management techniques. • Exceptional interpersonal skills and empathy for fostering client empowerment. • Proven ability to collaborate with multidisciplinary teams effectively. • Ability to coordinate placements and transitions within healthcare facilities. • Commitment to advocating for clients’ rights and promoting well-being. • No licensure required for the position.
