You’ll work closely with patients, caregivers, and their care teams to improve communication, cognitive-communication skills, swallowing safety, functional independence, and quality of life. Your responsibilities may include: • Assessing communication, speech, language, cognitive-communication, and swallowing abilities • Providing skilled speech, language, cognitive-communication, and swallowing interventions • Educating patients and caregivers regarding communication strategies, swallowing safety, and appropriate compensatory techniques • Monitoring patient progress and modifying treatment plans as needed • Collaborating with physicians, nurses, occupational therapists, physical therapists, and other members of the care team • Completing accurate and timely electronic documentation Home care offers a unique opportunity to get to know your patients and make a meaningful difference in their daily lives.
