Are you a nurse looking for an opportunity to use your clinical knowledge without working in a traditional patient-facing nursing role?
Assistance Plus is seeking a Nurse Care Manager to join our Behavioral Health team. This is primarily an office-based, behind-the-scenes care coordination position focused on helping individuals receiving care management services get their medical and behavioral health needs addressed.
This position works under the direction of the Director of Behavioral Health and collaborates with a multidisciplinary team, including primary care providers, behavioral health providers, care managers, and other healthcare professionals. The Nurse Care Manager helps connect the pieces of a client's care, identify needed follow-up, and promote a whole-person approach to health.
Position Overview:
As the Nurse Care Manager, you'll play an important role in coordinating medical and behavioral healthcare for individuals receiving BHH services.
Responsibilities include:
Using HealthInfoNet (HIN) to monitor healthcare activity and identify important follow-up needs.
Following up with clients, care coordinators, primary care providers, and other members of the care team following HIN notifications.
Providing physical and behavioral health consultation and helping coordinate services identified in clients' Plans of Care.
Supporting clients and care coordinators with identifying potential medication side effects and coordinating concerns with healthcare providers.
Collaborating with primary care providers, psychiatric providers and consultants, care coordinators, and the Behavioral Health leadership team.
Obtaining and reviewing important healthcare information, including diagnoses, laboratory information, physicals, hospitalizations, and discharge information.
Providing education and guidance to staff and clients regarding medical and psychiatric needs.
Preparing case reviews and participating in multidisciplinary consultation.
Developing, reviewing, updating, and overseeing policies and procedures related to medications, psychiatric medications, and medical treatments.
Monitoring performance measures and helping the program meet MaineCare Section 92 performance requirements and Pay for Performance benchmarks.
Completing required documentation within the agency's electronic health record.
Participating in treatment planning and team meetings as needed.
If you enjoy nursing but are looking for something outside of a traditional clinical or bedside environment, this position offers a different way to use your experience. You'll spend your time coordinating care, communicating with clients and providers, reviewing healthcare information, identifying needed follow-up, and helping a multidisciplinary team provide whole-person care.
You can make an impact on clients' health and quality of care while working in a professional office environment with part-time or full-time scheduling options and flexibility to work from home when needed.
