Assess patient needs, manage symptoms, and coordinate care with the interdisciplinary team to support patients and caregivers. On-Call Registered Nurse (RN) – Weekends Saturday and Sunday | 8:00 AM–8:00 PM | Plus one 8-hour weekday shift | Holiday rotation Provide hospice nursing support to patients and families during weekend and after-hours periods in home and facility-based settings, including private residences, skilled nursing facilities, assisted living facilities, and group homes. Responsibilities Responsibilities will vary depending on the Registered Nurse position and schedule and may include: • Provide compassionate, individualized hospice and palliative nursing care during assigned evening, weekend, and on-call hours. • Respond to patient and family needs in home care and facility-based settings. • Perform comprehensive nursing assessments addressing physical, psychosocial, environmental, safety, emotional, and spiritual needs. • Assess and manage pain, symptoms, and changes in patient condition. • Provide appropriate nursing interventions based on patient needs, physician orders, and established plans of care. • Develop, implement, and update individualized plans of care as needed. • Educate and support patients, families, and caregivers regarding symptom management, medications, and hospice services. • Demonstrate knowledge of hospice levels of care, including routine home care, continuous care, general inpatient care, and respite care. • Collaborate with physicians, interdisciplinary team members, and facility staff to coordinate patient care. • Provide appropriate shift-to-shift reporting and communicate important changes in patient condition. • Maintain accurate and timely electronic medical records and clinical documentation. • Follow established clinical, patient safety, privacy, and infection-control standards. • Travel to patient homes and healthcare facilities throughout the assigned service area. Qualifications • Graduate of an accredited school of nursing. • Current Registered Nurse (RN) license in the State of New York. • Valid driver's license and access to a vehicle for community travel. • One year of professional nursing experience preferred. • Ability to assess patient needs, respond to changes in condition, and coordinate care across home and facility-based settings.
