You'll manage symptoms, coordinate care, and give families the support and guidance they need during one of life's most meaningful transitions. Responsibilities: • Complete initial and ongoing comprehensive assessments of patients' physical, emotional, and psychosocial needs • Develop, implement, and update individualized plan of care in partnership with patients and families • Provide skilled nursing care, including pain and symptom management, medication administration, and comfort measures • Educate patients and caregivers on disease progression, symptom management, medications, and what to expect at end of life • Coordinate care across your caseload and serve as the primary liaison between patients, families, physicians, and the care team • Participate in interdisciplinary group (IDG) meetings to ensure comprehensive, holistic care Qualifications: • Active Registered Nurse (RN) license in the state you work in • Proficiency with electronic medical records (EMR) • Strong organizational, communication, and critical-thinking skills • Ability to work independently while maintaining high standards of care • A compassionate, patient- and family-centered approach • Valid driver's license and reliable vehicle with current insurance Benefits: • Competitive salary • Fleet car program with fuel and insurance covered, for business and personal use *Based on eligibility • Medical, dental, and vision insurance • Life insurance, Long term/Short term disability insurance • Generous PTO package • 401(k) with company match Join Our Team If you're ready to do meaningful work with a team that supports you, we'd love to hear from you.
