Employment Type: Full time Shift: Rotating Shift Description: Position Purpose Responsible for providing pharmacy care to support operations, clinical, retail, and/or specialty pharmacy services in the acute and/or ambulatory care areas. Responsible for ensuring performance excellence in terms of quality, safety, service, affordability, and technology. Serves as knowledge expert in specialty-based programs, education resource, role model and mentor. Coordinates/leads unit, program, or service scope of responsibilities, including management of human resources. What you will do • Lead quality and compliance programs and provide subject matter expertise (e.g., compliance, compounding, diversion, quality) • Design and implement pharmacy-based and regional performance improvement initiatives to ensure safe use of medications • Provide oversight and guidance for medication order review, preparation, distribution, and dispensing processes • Monitor and evaluate drug therapies to promote optimal drug use and evidence-based practices to reduce risks of drug misadventures • Lead medication safety measures and efficient operational processes • Provide drug information and therapeutic recommendations based on patient specific parameters and conditions • Perform medication therapy management (MTM) and comprehensive medication reviews (CMR) • Lead training, development, mentoring and assessment of competencies (e.g., Residency Preceptor) Minimum Qualifications • Bachelor’s Degree in Pharmacy, Doctor of Pharmacy (Pharm.D.) preferred • 3 years’ experience as a pharmacist, pharmacy residency preferred; internal or external certifications; Board of Pharmacy Specialists certification preferred • CT State Pharmacist License Position Highlights and Benefits • Full time 40hrs M-F days • Excellent benefits - starting day 1!
