Description Fresh Perspective Home Care is seeking a compassionate and organized Licensed Practical Nurse (LPN) to join our team in a hybrid role supporting both field and administrative operations. This position works directly with nursing clients and caregivers by conducting client assessments, developing and updating care plans, providing caregiver training, completing performance evaluations, and ensuring exceptional quality of care.
