Genesis Home Care | South Zanesville, OH Schedule: Monday–Friday | 8:00 AM–5:00 PM (with on call requirements) ** $7,500 Sign On Bonus** Genesis Home Care is seeking a compassionate and dependable Licensed Practical Nurse (LPN) to provide skilled nursing care to patients in the comfort of their homes. What You’ll Do • Provide skilled nursing care according to physician orders and RN-developed plans of care • Assess and monitor patient conditions and communicate changes to the RNCM • Administer medications and treatments as prescribed • Educate patients and families on disease management, medications, safety, and preventative care • Involve patients and families in their plan of care • Complete accurate and timely clinical documentation in accordance with agency and Medicare requirements • Communicate with the care team and scheduler regarding patient needs and schedule changes • Participate in on-call coverage, case conferences, in-service training, and required meetings • Provide care within the LPN scope of practice and applicable state and federal regulations • Travel to patients’ homes within the assigned geographic area What We’re Looking For • Current, active LPN license in the state of Ohio • Minimum of 1 year of acute care LPN experience required; 2 years preferred • Home health experience preferred • Strong communication and interpersonal skills • Compassionate, patient-focused approach • Ability to work independently, prioritize responsibilities, and make appropriate nursing judgments • Strong attention to detail and reliable work ethic • Ability to work effectively as part of a team • Valid driver’s license and auto insurance with your name listed as a driver Education & Credentials • Graduate of an approved practical nursing program • Current, active LPN license in the state of Ohio Join Genesis Home Care Make a difference by providing meaningful, one-on-one care to patients where they are most comfortable — at home! #INDGHC8 We’ll help you put your passion for patient care to work.
