CNA (Home Health) Shift Available: PRN Service Areas for Coverage: North, Northeast, Northwest Responsibilities: • Provide exceptional care in coordination with the assigned nursing care team. • Maintain patient records that include any changes in doctor’s orders, diet, or personal living activities. • Help with patient meals, feeding, and basic activities of daily living. • Deliver weekly reports to the case manager and review any important changes in patient conditions. • Educate patients and family members. • Prepare timely, complete, and accurate clinical notes. • Communicate effectively with healthcare providers and other care team members.
