To coordinate clinical services of Evening Clinic patients. 1. Provide coordination of services in accordance with psychiatric and psychosocial assessment and diagnosis. 2. Coordinate with other members of multi-disciplinary team in developing and implementing treatment plan. 3. Conduct individual and/or group treatment sessions for patients and families. 4. Document in medical record diagnostic impressions of patient status and current progress. 5. Coordinate patient/family education; encourage participation in care and consumer involvement. The salary range for the role is $58,661.00 - $81,675.00 Annually. Actual salaries depend on a variety of factors, including experience, education, and operational need.
