Job Description Spectrum Healthcare Resources has an excellent opportunity for a Full time Registered Nurse - Behavioral Health Case Manager at the Fairfax Ambulatory Care Clinic in Fairfax, VA. This position offers: • Full-Time Schedule: 40 hours/week • Work Schedule: Monday - Friday 0730-1600 • Outpatient healthcare setting • Work-life Balance: No on-call, weekend or holiday requirements, with 11 paid federal holidays off • Pay range: $38.95-$43.88 per hour • Benefits: Medical, dental, vision insurance; paid time off (PTO), sick leave, life insurance and more. • Meaningful Work: Opportunity to provide healthcare resources to our nation's military community. Essential Functions: • Triage patients by gathering relevant information to assess the nature and urgency of behavioral health concerns. • Assist patients in scheduling appointments within the direct and purchased care behavioral health system. • Facilitate emergent care coordination and transfers for inpatient psychiatric hospitalization. • Collaborate with other healthcare team members to develop and coordinate comprehensive patient care plans. • Work closely with the behavioral health provider team to monitor, track, and manage patients identified as high risk. • Serve as a resource person and facilitator for patients, helping them access available community resources. • Collaborate with Health Center Case Management staff to support ongoing case management efforts for behavioral health patients requiring case management tracking and assistance. Job Requirements: • Must be a U.S. Citizen. • Bachelor of Science in Nursing (BSN) required • Minimum of one year of Behavioral Health or Case Manager RN experience, preferably within the last five years Ready to seize this opportunity?
