The RN Care Transition Coordinator position is responsible for providing care transition services for Providence hospitalized patients. The Coordinator works with the Providence physician team and hospital discharge planning staff to ensure patients are successfully discharged from the hospital with no disruption in services. The position encompasses the roles of nursing assessment, liaison abilities, communication, education and team/family support. Required Qualifications: • Coursework/Training Graduate from an accredited school of Nursing. • Upon hire: Washington Registered Nurse License Preferred Qualifications: • Bachelor's Degree in nursing. • 1 year Clinical experience. • Experience in providing patient and staff education. • Experience in conflict resolution.
