We are seeking a dedicated and detail-oriented MDS Coordinator (Registered Nurse/RN) to join one of our partner facilities in Wauneta, Nebraska. This role offers an experienced RN the chance to coordinate care, conduct clinical assessments, and collaborate with an interdisciplinary team to support quality resident care. Key Responsibilities: • Complete and coordinate Minimum Data Set (MDS) assessments in accordance with applicable regulations and required timelines. • Develop, review, and update individualized resident care plans. • Collaborate with nursing staff, therapy, dietary, social services, and other members of the interdisciplinary team. • Ensure accurate clinical documentation and compliance with federal and state requirements. • Monitor assessment schedules and deadlines to support timely completion and submission. • Participate in care plan meetings, resident assessments, and quality improvement initiatives. • Support effective communication and coordination to promote positive resident outcomes. Qualifications: • Current, active, unencumbered Nebraska Registered Nurse (RN) license. • Minimum of 3 years of RN experience, including at least 1 year of supervisory experience. • Strong organizational, communication, and teamwork skills. • Resident Assessment Coordinator training required within 6 months of hire. Preferred Skills: • Previous MDS experience preferred. • Certification preferred.
