Job Title Licensed Practical Nurse (LPN) Location St. Albans, VT | Skilled Nursing Facility Schedule / Job Type Contract | Midshift | Every other weekend required Key Responsibilities • Provide direct patient care in a long-term care / skilled nursing facility setting • Administer medications and treatments as directed by the care plan • Monitor and document patient conditions, progress, and responses to care • Collaborate with interdisciplinary care teams to support resident health and safety • Maintain valid state LPN licensure and adhere to all facility compliance requirements Required Qualifications • Active LPN license in the state of Vermont • Must be a local candidate — locals only eligible for this assignment • Must have reliable personal transportation • Must present a valid state or federal issued photo ID and wear a photo badge at all times while on assignment • Must have completed the original or updated COVID-19 vaccine series, or provide a documented religious and/or medical exemption from employer Preferred Experience / Skills • Prior experience in a long-term care or skilled nursing facility environment • Familiarity with electronic health records and clinical documentation Why Join Us • Opportunity to make a meaningful impact in a long-term care community serving residents who depend on consistent, compassionate nursing care • Overtime may be available depending on facility needs
