A Director of Case Management opportunity is available in Sanford, FL at a 221-bed regional medical center — including 199 acute care beds and 22 rehab beds — that treats approximately 43,139 patients annually and holds a Level II Trauma designation. The role. Reporting to the Division VP of Case Management, this director leads a team of 13–15 employees, including a Manager and Lead, and carries full accountability for all operational aspects of the hospital's case management department. The director directs and evaluates the case management model, staffing skill mix, use of information technologies, onboarding, and staff competencies to achieve performance and quality objectives. A core focus is ensuring adherence to care coordination and discharge planning processes — maintaining timeliness, quality, and proper documentation — while partnering with the facility CFO and Division CM Leadership to meet established goals. The director assesses and continuously improves departmental performance by analyzing operational data and implementing sustainable performance improvement activities, and builds strong working relationships with key stakeholders including the CMO, CNO, ancillary service leaders, and Ethics & Compliance. Success in this role requires executive presence, change management capability, and the ability to manage multiple priorities with minimal supervision across a broad set of interdisciplinary and external stakeholders. What we're looking for. • 2+ years of RECENT (within the last year) acute care Case Management leadership experience required • Bachelor's degree required • Master's degree in Nursing, Health Administration, or Business Administration, preferred • (RN) Registered Nurse, or (LCSW) Clinical Social Worker, (RN) Registered Nurse preferred Schedule. Days, Monday–Friday, 7–10 hours per shift, with possible on-call rotations or occasional late nights and extended days. Compensation. Based on years of experience: 1-3 years, $102,107 - $115,000; 3-5+ years, $120,000 - $135,000. Plus 17.5% annual bonus. Sign-on bonus: case by case. Relocation assistance: case by case.
